To provide exceptional care to each patient, a scheduled appointment is required. Please call our office to schedule an appointment. 

Canceling appointments:
Please provide our office with 24 hours (advance) notice for appointment cancellations. 

Co-payments and patient balances are due at the time of service. Per St. Anthony's Physician Organization (SAPO) and our Central Billing Office (CBO), there is a returned check fee of $30.00. 

We make every effort to contact you within a reasonable time regarding your test results. If you have not heard from our office within two weeks from the date of service, please contact our office. 

In an effort to contact you and provide important services, please notify our office with any changes in name, address, phone number, pharmacy, or insurance information.

What to bring to your appointment:

Please bring your MRI, CT, Myelogram, and/or x-rays with you; a CD copy is acceptable.  Films performed at St. Anthony’s Medical Center are accessible from our office.  It is usually necessary to contact the location where the tests were completed.  If you have had a prior neurosurgery it is important to bring records with you (e.g. operative reports). 

Please have films available prior to the date of your appointment.  Do not depend on a doctor's office, courier service, or testing center, to send films.  If you do not arrive with films it may be necessary to reschedule your appointment.

Diagnostic tests, such as blood work, EMG/Nerve Conduction Study, etc., do not require films; however, the results are equally important. If you have had a recent test in these areas, please bring those reports with you as well. 

In addition, please have all your paperwork completed at the time of your appointment.  Please do not mail or fax the forms to our office.  You will need to bring your insurance card, driver’s license, and co-pay if applicable.
